Job Information


  • Under the general guidance of the Director of Outpatient Services, this position will be the principal point of contact for chronic pain management/chronically ill patients, caregivers and providers to provide resources and assistance with accessing clinical and supportive care within the Healthcare District and/or community.
  • Cultivates and supports primary and specialty care providers with timely communication, inquiry, follow-up and integration of information into the plan of care.
  • Assists patient in understanding their diagnosis, treatment options and available resources.
  • Facilitates outreach efforts to establish and maintain positive working relationships with key community partners.
  • Works collaboratively to collect data, track outcomes and support strategic planning processes related to PRIME project.

  • Highly organized with ability to keep accurate notes and records
  • Ability to maintain confidentiality
  • Computer literacy in Microsoft Office
  • Willingness to learn facility electronic health system
  • Must have the ability to work collaboratively with physicians and other members of the health care team
  • Must be able to withstand stressful situations
  • Requires problem solving, decision making and critical thinking
  • Must be able to work in a self-directed environment, with an ability to work and lead teams
  • Knowledge of basic medical terminology
  • Knowledge of local resources and social welfare resources is desirable

Education and Training

  • Social Worker (MSW preferred)
  • Current BLS certification within 90 days of employment
  • Desire and experience working within a primary care clinic is highly preferred.
