Changes to Medical Records
In order to protect our patients and staff, there is a new procedure to request medical records:
If you need your medical records, please call or email, we will no longer accept requests in person for records.
Please call 909-878-8273 or email your detailed request with a copy of your ID to roi.requests@bvchd.com
Please include type of records and date range in request.
Patients can have medical records be sent by
• Secure email
• Fax to your doctor office
• Mail
Processing times may vary depending on the complexity of your request, however in most cases requests are completed within two to three business days from the date of request receipt.
