Job Information

SNF Responsibilities: 

  • Assume responsibility for the overall direction, coordination and evaluation of nursing care and services provided to the residents
  • Work closely with MDS Coordinator and certifies that all RAI are complete and transmitted to CMS
  • Ensures compliance with state and federal licensing regulations  and/or accreditation agencies within area of responsibility
  • Establishes departmental goals, objectives and budgets in accordance with BVCHD’s mission and strategic plan
  • Oversee the Director of Staff Development, and ensures the orientation and in-service program meet title 22 requirements
  • Knowledge of performance improvement and risk management principles

Med/Surg Responsibilities:

  • Responsible for planning, directing and participating in all aspects of nursing activities to provide optimum achievable nursing care
  • Responsible for scheduling, evaluation, and counseling of personnel, recommend hiring, promotions, transfers and/or termination in collaboration with the CNO
  • This position works closely with physicians, families and other health professionals in providing for patient care needs
Qualifications

Education:

  • Current California Registered Nurse License
  • BLS certification
  • ACLS certification preferred
  • PALS certification preferred
  • Basic Dysrhythmia certification preferred
  • Acute care nursing experience required
  • Previous experience in SNF setting in the last (24) months
  • Obtain DSD certificate within (6) months of hire

Responsibilities

  • Assess the condition, progress, needs, and problems of the patient upon admission and each shift thereafter
  • Establish nursing diagnosis, list patient problems, select appropriate interventions, establish goals, and document progress in medical record with ongoing assessment
  • Assists patients with activities of daily living within the scope of physician orders
  • Assists in maintaining efficiency of nursing unit and providing a clean, safe environment to assure optimal unit functioning and patient/employee safety
Qualifications

Education

  • Graduation from an accredited School of Nursing.
  • Requires 1 – 2 years med / surg, Telemetry or Critical Care nursing experience.
  • Current Basic Life Support (CPR) card.
  • Current Advanced Cardiac Life Support (ACLS) card
